Carpal Tunnel Syndrome: What It Is, Why It Happens, and How to Stop It Before It Starts

If you've ever woken up with numb, tingling fingers or noticed your grip weakening halfway through a workday, you're not imagining it. Carpal tunnel syndrome (CTS) is one of the most common nerve compression conditions we see in clinical and workplace settings.
What Is Carpal Tunnel Syndrome?
The carpal tunnel is a narrow passageway on the palm side of your wrist, formed by small wrist bones (the carpal bones) on one side and a tough band of connective tissue (the transverse carpal ligament) on the other. Through this tunnel runs the median nerve, along with nine tendons that flex your fingers and thumb.
There's very little spare room in that tunnel. When the tissues inside it swell, from repetitive movement, inflammation, fluid retention, or awkward positioning, pressure builds on the median nerve. That compression is what produces the hallmark symptoms of CTS.
Recognising the Symptoms
CTS tends to develop gradually, and symptoms are often worse at night or first thing in the morning. Common signs include:
Numbness or tingling in the thumb, index, middle, and half of the ring finger (notably sparing the little finger)
A sensation of swelling in the fingers, even when no visible swelling is present
Weakened grip strength or clumsiness, dropping objects, difficulty with buttons or jar lids
Aching that radiates up the forearm
Symptoms that improve temporarily when the hand is shaken out ("flick sign")
Left unaddressed, prolonged compression can lead to measurable muscle wasting at the base of the thumb and permanent nerve changes. Early intervention matters.
What Causes It — And Why the Workplace Matters
CTS is multifactorial. Some risk factors are non-modifiable — sex (it's more common in women), age, pregnancy, diabetes, and thyroid conditions all play a role. But a significant proportion of cases are strongly linked to modifiable occupational and postural factors, including:
Sustained wrist flexion or extension: common with poorly positioned keyboards, laptops without external peripherals, or workstations set up around the desk rather than the person
Repetitive gripping or pinching: checkout scanning, packing, assembly line work, hairdressing, and trades using vibrating tools
Awkward or static postures held for long period: particularly when combined with force
Poor forearm and wrist support: desks and chairs that leave wrists unsupported and angled downward or upward for hours at a time
This is precisely where ergonomics earns its place in the conversation. CTS is rarely caused by a single dramatic event, it's the cumulative product of thousands of small, repeated mechanical stresses. Change the mechanics, and you change the trajectory.
Diagnosis and Management
A physiotherapist or GP will typically assess through a combination of clinical tests, Tinel's sign (tapping over the nerve), Phalen's test (sustained wrist flexion), grip strength, and sensory mapping. Nerve conduction studies are used to confirm diagnosis and grade severity in more persistent cases.
Management is staged according to severity:
Conservative management (mild–moderate cases): activity modification, night splinting to keep the wrist neutral, nerve gliding exercises, anti-inflammatory strategies, and correction of the underlying ergonomic driver.
Medical management: corticosteroid injection to reduce inflammation around the nerve in cases that don't respond to conservative measures.
Surgical management: carpal tunnel release, reserved for moderate-to-severe cases with persistent symptoms, motor weakness, or nerve conduction evidence of significant compression. Recovery generally involves a graded return to function, and attention to the workstation or task setup that contributed in the first place, to prevent recurrence.
Why Prevention Beats Treatment
Treating CTS after it develops, whether conservatively or surgically, is manageable, but it's also disruptive: lost work capacity, rehabilitation time, and in more severe cases, extended recovery periods. The more strategic play is upstream: identifying and correcting the postures, equipment, and task design that put the median nerve under sustained pressure in the first place.
This is where most workplaces fall short, not from lack of awareness, but from lack of qualified assessment. A chair adjustment or a new mouse pad might offer marginal relief, but genuine risk reduction requires understanding the biomechanics of the individual worker, their tasks, and their equipment as a system.
